New Patient Registration

How to register with us

To register at Horizon Healthcare, each new registrant needs to complete a GMS1 form and the most appropriate registration form (links are available at the bottom of this page) or alternatively, registration packs can be collected from the practice.

Upon completion, these forms will need returning to the practice by emailing them to horizon.healthcare1@nhs.net or by dropping them back to the practice. You will also need to provide some photo identification via email or in person.

If you are on regular medication, it may be useful to send us a photo of your medication showing your details and the preparation, or bring the packaging to the practice in person.

Medical treatment is available from the date of registration. Please contact reception for further information.

Please download and complete ONE GMS1 form and ONE Registration form for each new registrant.

INCOMPLETE FORMS – Please check every question has been answered.

If you have NEVER had a UK GP before, we need the date you entered the UK.

If you get a message to call us about your registration it is usually because the NHS has a different address than the one you gave us, please call us urgently.

Note that by sending the form you will be transmitting information about yourself across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.

Alternatively you may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

Patients wishing to join our practice can do so by contacting the receptionists during normal working hours.  A registration health check, carried out by a practice nurse will be offered to you.

New patients who are on regular medication (including contraceptive pill) or having treatment for chronic conditions must make an appointment with the nurse/doctor before their prescriptions can be issued.

Medical treatment is available from the date of registration. Please contact reception for further information.
