Introduction to Live Well 

Leicester City Council’s Live Well service is designed to support the residents of Leicester to make healthier lifestyle choices and improve their overall well-being. Live Well provides a range of free and confidential services that empower individuals to take control of their health, manage long-term conditions, and make positive changes to their daily habits. 

This information pack presents an overview of the services we offer, the eligibility criteria, and how you can refer people into a Live Well service. 

Services We Offer 

Live Well provides a variety of services aimed at improving the health and well-being of individuals. We offer personalised one-to-one support from trained advisors to help clients achieve their health goals. Specialist advisors can assist with smoking cessation, increasing physical activity and healthier eating, and reducing alcohol consumption. 

Our services include: 

  1. Stop Smoking A 12-week programme providing support for individuals who want to quit smoking tobacco. Our service offers behavioural therapy and free nicotine replacement products to help people become smoke-free. 
  2. Physical Activity and Healthier Eating A 12-week programme for adults looking to increase their fitness levels and develop an active lifestyle, lose weight, and adopt healthier eating habits. We offer free access to local leisure centre facilities and fitness sessions run by our specialist advisors, as well as educational sessions providing nutritional guidance for individuals who need support in making healthier dietary choices. 
  3. Live Well Walk More 

A walking group programme which offers a variety of volunteer-led walks across Leicester. Our weekly walk timetable offers something for everyone with short, medium and long-distance walks covering areas including Braunstone, Beaumont Leys, New Parks, the city centre, and more. 

Live Well Walk More also includes our monthly Buggy Walks for families across the city, led by our expert healthy lifestyle advisors. These free walks are a great way for young families to get outside, connect with the local community, and enjoy a friendly stroll with other parents. Plus, we’re offering storytelling, arts and crafts and free hot drinks. 

  1. Steady Steps 

A 24-week programme tailored to those who have previously fallen or worry about falling. The programme is specifically designed to help improve balance and stability and prevent future falls, through adapted exercises that help to build mobility and independence. 

  1. Reducing Alcohol Turning Point is Leicester’s integrated substance use service. They work with anyone who is affected by substance use and wants help to make a change to their life. They offer a range of support and treatment options and will aid clients in finding the right care for them, including options such as one to one support and/or group support sessions, and opiate substitute prescribing. 

Who is Eligible for Live Well Services? 

Live Well services are generally available to residents of Leicester City. Eligibility for specific services may depend on the individual’s health needs and circumstances. People who may benefit from our services include: 

  • People who want support to stop smoking. 
  • Individuals looking to improve their physical activity levels. 
  • Adults who wish to lose weight or manage their weight more effectively. 
  • Anyone seeking guidance on improving their diet and nutrition. 
  • Those with long-term conditions such as diabetes, hypertension, or mental health conditions. 

Stop Smoking The Stop Smoking service is available to all tobacco smokers (including shisha) or chewers who are aged 12 or above. The Stop Smoking service is not only open to Leicester residents, but also to anyone who is employed in the city and/or registered to a GP practice in the city – even if they are resident outside of Leicester. 

Physical Activity and Healthier Eating The Physical Activity service is available to inactive people aged 16 or above with a chronic health condition for which they require medication, who complete less than 30 minutes of vigorous physical activity per week. A referral from a healthcare professional is required to qualify for the Physical Activity programme. Clients must be resident in Leicester. 

Live Well Walk More 

Anyone over the age of 16 can join our walks, however, if anyone under the age of 16 would like to join our walk they must be accompanied by an adult. If anyone would like to bring a dog on the walk, they can if the dog is kept on a short lead for the duration of the walk. There are no residency criteria. 

Steady Steps 

The Steady Steps service is available to people over the age of 65, who are unsteady on their feet and would like to improve their balance, or who have previously fallen. The Steady Steps programme is open to residents in Leicester and Leicestershire. 

Reducing Alcohol 

Turning Point offer services to individuals regardless of their age or current health status, whether they are living with a chronic condition or simply looking to improve their overall well-being. 

How to Refer into Live Well 

Referring someone into a Live Well service is easy. There are several ways you can refer: 

  1. GP Referral If you have a patient who would benefit from Live Well services, please complete and submit an LLR Integrated Care Board PRISM Referral Form directly to Live Well via SystmOne. Please forward referrals via nhs.net email to: leicestercc.livewell@nhs.net. Once we receive a referral, we will contact the client to arrange an initial consultation. 
  2. Online Referral Form Complete the online professional referral form on our website (Professional Referrals – Live Well Leicester). 
  3. Referral by Phone Call our dedicated Hub line on 0116 454 4000. Our team can assist with completing the referral over the phone. 
  4. Self-Referral Clients can also refer themselves directly to Live Well through the self-assessment form on our website – Personal Details | Assessment Platform, or by calling our helpline. We encourage clients to take the initiative in accessing support. 

Once referred, clients will be assessed by one of our advisors who will work with them to create a personalised support plan based on their needs. 

How We Support Our Clients 

Once a referral is made, the Live Well team will: 

  • Contact the client to arrange an initial consultation. 
  • Discuss their health goals, identify barriers to change, and agree on a tailored plan of action. 
  • Offer regular follow-up support via telephone or face-to-face appointments. 
  • Provide resources, tools, and motivation to help clients stay on track with their health and well-being goals. 

Our service is completely confidential, and all data provided is handled in accordance with data protection regulations.
